What Actually Improves Blood Sugar, Ranked

Any honest discussion of a blood sugar supplement has to sit inside a picture of what else is available, because the comparison is not close.

1. Prescribed medication, where indicated

Nothing else on this list has the evidence base that diabetes medications have. Metformin and the drugs developed since have been tested in large trials measuring outcomes that actually matter — complications, hospitalisations, mortality — not just changes in a number.

If medication has been prescribed for you, it is doing more than any supplement can, and it should not be reduced or stopped without your doctor.

2. Weight loss, where relevant

The most powerful lifestyle intervention available for type 2 diabetes and prediabetes. Losing excess weight improves insulin sensitivity substantially, and research has shown that in some people substantial weight loss can bring type 2 diabetes into remission.

No supplement has ever demonstrated anything comparable. It is difficult, and it is difficult in a way that produces real results rather than in a way that merely feels virtuous.

3. Physical activity

Muscle takes up glucose during and after exercise, partly through routes that do not depend on insulin — which is why activity helps even when insulin resistance is present.

Both aerobic activity and resistance training are useful and the combination is better than either alone. Effects on insulin sensitivity begin quickly, within days rather than months, which makes this one of the more immediately rewarding changes available.

4. Dietary change

Particularly the quantity and type of carbohydrate, and overall energy intake where weight is a factor.

Deliberately, we are not prescribing a specific diet. The evidence supports several approaches, and the version that works is the one a particular person can sustain. A dietitian will get you further than any general rule, and referrals are more available than people assume.

5. Sleep and stress

Both genuinely affect glucose regulation. Poor sleep impairs insulin sensitivity measurably, and stress hormones raise blood glucose directly. These are consistently underrated because they are harder to sell than a bottle.

6. Structured prevention programmes

Worth naming separately. Programmes based on the National Diabetes Prevention Program — built around modest weight loss and regular activity with structured support — have been shown in large trials to substantially reduce progression from prediabetes to type 2 diabetes. Versions are widely available, often free or low cost.

7. Supplements

Here, at the bottom. Some ingredients have genuine research behind them at particular doses, and berberine is the most interesting of them. But the effect sizes are modest, the trials are smaller and shorter, and outcome data of the kind medications have simply does not exist.

Not an argument that supplements are worthless

It is an argument about order and proportion. Someone doing the first six and adding a supplement with their care team's knowledge is in a sensible position. Someone taking a supplement instead of the first six has chosen the weakest option on the list — and may feel the problem is being addressed when it is not.

The cost comparison worth making

A six-bottle supplement kit at $294 is roughly what a few sessions with a dietitian cost, or a year of a low-cost gym membership, or a decent glucose meter with a long supply of strips. Two of those three build something you keep.

And the thing that costs nothing

Getting tested, and going to your appointments. Diabetes complications develop over years without symptoms, which is exactly why the people who do best are the ones whose numbers are being watched.
